If you registered and paid for the field trip to Sandy Point State Park, Annapolis on the 26th of May, please stop by the classroom of the teacher you paid and registered with and pick up the Emergency Medical Treatment Authorization form and Parent waiver water activities at amusement park forms. These forms must be completed by your parents/ guardians and returned to the classroom teacher you registered with on or before Friday the 20th. You cannot  be allowed to embark on this trip if these forms are not completed and submitted
